Mae’r categorïau o wybodaeth disgyblion yr ydym yn ei phrosesu yn cynnwys y rhestr isod, nad yw’n gyflawn nac yn derfynol:
- manylion adnabod a chyswllt personol (megis enw, rhif disgybl unigryw, manylion cyswllt a chyfeiriad)
- nodweddion (megis ethnigrwydd, iaith a chymhwyster prydau ysgol am ddim)
- gwybodaeth am ddiogelu (megis gorchmynion llys ac ymgysylltiad proffesiynol)
- anghenion addysgol arbennig
- meddygol a gweinyddol (megis gwybodaeth meddyg, iechyd y plentyn, iechyd deintyddol, alergeddau, meddyginiaeth ac anghenion dietegol)

Rydym hefyd wrthi’n diweddaru gwefan yr ysgol o ran y math o wybodaeth yr ydym yn ei phrosesu.
Pam yr ydym yn casglu ac yn defnyddio gwybodaeth am ddisgyblion
Rydym yn casglu ac yn defnyddio gwybodaeth disgyblion i’r dibenion canlynol:
- i gefnogi dysgu’r disgyblion
- i fonitro ac adrodd ar gynnydd disgyblion o ran cyrhaeddiad
- i ddarparu gofal bugeiliol priodol
- i asesu ansawdd ein gwasanaethau
- i gadw plant yn ddiogel (alergeddau bwyd neu fanylion cyswllt mewn argyfwng)
- i ddiwallu’r dyletswyddau casglu data statudol a bennwyd gan Lywodraeth Cymru.
- i gyfathrebu’n effeithiol gyda rhieni a gofalwyr
- i alluogi disgyblion i symud o un lleoliad addysgol i un arall yn ddi-dor,
O dan y Rheoliad Diogelu Data Cyffredinol y seiliau cyfreithlon y byddwn yn dibynnu arnynt ar gyfer prosesu data disgyblion fydd y canlynol fel arfer:
- Fel rheolwr y data, mae’r ysgol yn defnyddio’r wybodaeth a geir i’r dibenion a restrir uchod er mwyn ein galluogi i gynnal y prosesu data sy’n angenrheidiol ar gyfer perfformio tasg a gyflawnir er budd y cyhoedd ac wrth arfer awdurdod swyddogol neu os oes arnom ddyletswydd gyfreithiol i’w phrosesu.
- Yn ychwanegol, mewn perthynas ag unrhyw ‘ddata categori arbennig’ lle mae’r prosesu’n angenrheidiol am resymau o fudd cyhoeddus sylweddol; neu lle cawsom ganiatâd clir a phenodol.
Sut yr ydym yn casglu gwybodaeth am ddisgyblion
Rydym yn casglu gwybodaeth am ddisgyblion drwy ffurflenni derbyn neu ffurflenni cofrestru sy’n cael eu llenwi ddechau’r flwyddyn ysgol, drwy ‘ffeiliau ‘trosglwyddiad cyffredin’ neu drosglwyddiad ffeil o ysgol flaenorol.
Mae data disgyblion yn hollbwysig at ddefnydd gweithredol yr ysgol. Tra bo’r rhan fwyaf o wybodaeth disgyblion a roddir i ni yn orfodol, gofynnir am rywfaint o wybodaeth ar sail wirfoddol.
Er mwyn cydymffurfio â’r ddeddfwriaeth diogelu data, byddwn yn rhoi gwybod i chi ar bwynt casglu’r data a oes angen i chi roi gwybodaeth benodol am ddisgybl i ni neu a oes gennych ddewis yn hyn o beth.
Sut yr ydym yn cadw data disgyblion
Rydym yn cadw data disgyblion yn ddiogel am y cyfnod penodol o amser a ddangosir yn ein hamserlen cadw data. Am ragor o wybodaeth am ein hamserlen cadw data a sut yr ydym yn cadw ein data’n ddiogel, ewch i wefan yr ysgol i weld y wybodaeth ddiweddaraf. Er enghraifft mae’n rhaid i ni gadw rhai mathau penodol o wybodaeth addysgol tan ben-blwydd y disgybl yn 25.
Gyda phwy y byddwn yn rhannu gwybodaeth am ddisgyblion
Rydym yn rhannu gwybodaeth berthnasol am ddisgyblion fel mater o drefn gydag:
- ysgolion y bydd disgyblion yn eu mynychu ar ôl ein gadael ni
- ein hawdurdod lleol
- Llywodraeth Cymru
- GIG/Nyrs ysgol; naill ai yn uniongyrchol neu drwy’r awdurdod lleol ar gyfer rhaglenni imiwneiddio.
Pam yr ydym yn rhannu gwybodaeth am ddisgyblion yn rheolaidd
Nid ydym yn rhannu gwybodaeth am ein disgyblion gydag unrhyw un heb ganiatâd oni bai fod y gyfraith a’n polisïau ni yn caniatáu i ni wneud hynny. Ble bo hynny’n briodol byddwn yn anelu at fod â Phrotocol Rhannu Gwybodaeth ffurfiol yn ei le ac rydym wedi ymrwymo i ddatblygu hwn yn unol â’r safonau cenedlaethol a bennwyd yng Nghytundeb Rhannu Gwybodaeth Bersonol Cymru (WASPI), gweler:www.waspi.org
Llywodraeth Cymru
Mae Llywodraeth Cymru yn casglu data personol drwy gydol cyfnod plentyn yn yr ysgol gan leoliadau addysgol ac awdurdodau lleol drwy amryw gasgliadau data statudol megis:
- Cyfrifiad Ysgol Blynyddol ar Lefel Disgyblion (PLASC)
- Casgliad lefel disgyblion a addysgir rywle heblaw’r ysgol (EOTAS_
- Casgliadau Data Cenedlaethol
- Casgliadau presenoldeb
- Casgliadau data Profion Cenedlaethol Cymru (PCC)
- Casgliadau ôl 16
Yn ychwanegol at y data a gesglir fel rhan o PLASC, mae Llywodraeth Cymru a’r Awdurdodau lleol hefyd yn derbyn gwybodaeth am asesiadau’r Cwricwlwm Cenedlaethol, canlyniadau arholiadau cyhoeddus a data presenoldeb ar lefel disgyblion unigol sy’n dod gan ysgolion a/neu gyrff dyfarnu (e.e. CBAC). Mae’n ofynnol i ni rannu gwybodaeth ein disgyblion gyda Llywodraeth Cymru naill ai’n uniongyrchol neu drwy ein hawdurdod lleol i ddibenion y casgliadau data hynny, o dan Reoliadau Gwybodaeth Disgyblion (Cymru) 2011.
Pam yr ydym yn rhannu’r data hwn gyda Llywodraeth Cymru?
Mae’r data disgyblion yr ydym yn ei rhannu’n gyfreithlon gyda Llywodraeth Cymru drwy gasgliadau data:
- yn tanategu cyllid ysgolion, a gaiff ei gyfrifo yn seiliedig ar nifer y plant a’u nodweddion ym mhob ysgol.
- yn hysbysu monitro polisïau addysgol ‘byrdymor’ ac atebolrwydd ysgolion ac ymyrraeth (er enghraifft canlyniadau TGAU ysgolion neu fesuryddion cynnydd disgyblion).
- yn cefnogi ymchwil a monitro polisi addysgol ‘tymor hirach’ (er enghraifft sut y mae rhai dewisiadau pynciau yn effeithio ar addysg neu enillion y tu hwnt i’r ysgol)

Privacy Notice (Primary Schools)
The categories of pupil information that we process include the following non-exhaustive list:
- personal identifiers and contacts (such as name, unique pupil number, contact details and address)
- characteristics (such as ethnicity, language, and free school meal eligibility)
- safeguarding information (such as court orders and professional involvement)
- special educational needs
- medical and administration (such as doctors information, child health, dental health, allergies, medication and dietary requirements)
- attendance (such as sessions attended, number of absences, absence reasons and any previous schools attended)
- assessment and attainment (such as key stage 1 and phonics results)
- behavioural information (such as exclusions and any relevant alternative provision put in place)
- information about attendance on school trips and activities (safety issues and risks for keeping a pupil safe, emergency contact details; next of kin)
- food allergies (for catering)
Why we collect and use pupil information
We collect and use pupil information, for the following purposes:
- to support pupil learning
- to monitor and report on pupil attainment progress
- to provide appropriate pastoral care
- to assess the quality of our services
- to keep children safe (food allergies, or emergency contact details)
- to meet the statutory duties placed upon us by the Welsh Government with regard to data collection.
- to communicate effectively with parents and carers.
- to enable pupils to move from one educational setting to another seamlessly
Under the General Data Protection Regulation (GDPR), the lawful bases we rely on for processing pupil information will usually be the following:
- As data controller, the School uses the information received for the purposes listed above to enable us to carry out data processing necessary for the performance of a task carried out in the public interest and in the exercise of official authority or where we have a legal obligation to process.
- In addition, concerning any ‘special category data’, where the processing is necessary for reasons of substantial public interest; or where we have obtained explicit consent.
How we collect pupil information
We collect pupil information via admission forms or registration forms completed at the start of school years; via ‘common transfer files’, or file transfer from a previous school.
Pupil data is essential for the schools’ operational use. Whilst the majority of pupil information you provide to us is mandatory, some of it requested on a voluntary basis. In order to comply with the data protection legislation, we will inform you at the point of collection, whether you are required to provide certain pupil information to us or if you have a choice in this.
How we store pupil data
We hold pupil data securely for the set amount of time shown in our data retention schedule. For more information on our data retention schedule and how we keep your data safe, please visit the school website for the latest information. We are required to retain certain educational information until a pupil’s 25th birthday for example.
Who we share pupil information with
We routinely share relevant pupil information with:
- schools that the pupils attend after leaving us
- our local authority
- Welsh Government
- NHS /school nurse , either directly or via the local authority such as for immunisation programmes.
Why we regularly share pupil information
We do not share information about our pupils with anyone without consent unless the law and our policies allow us to do so. Where appropriate we will aim to have a formal Information Sharing Protocol in place and have made a commitment to developing these in accordance with national standards laid down by the Welsh Government’s Wales Accord on the Sharing of Personal Information (WASPI) see www.waspi.org
Welsh Government
The Welsh Government collects personal data throughout a pupil’s school life from educational settings and local authorities via various statutory data collections such as:
- Pupil Level Annual School Census (PLASC)
- Educated other than at school (EOTAS) pupil level collection
- National data collection (NDC)
- Attendance collection
- Welsh National Tests (WNT) data collection
- Post 16 data collection
In addition to the data collected as part of PLASC, the Welsh Government and Local Authorities also receives information regarding National Curriculum assessments, public examination results, and attendance data at individual pupil level which comes from Schools and /or Awarding Bodies (e.g. WJEC).
We are required to share information about our pupils with the Welsh Government either directly or via our local authority for the purpose of those data collections, under The Pupil Information (Wales) Regulations 2011.
Why do we share this data with the Welsh Government?
The pupil data that we lawfully share with the Welsh Government through data collections:
- underpins school funding, which is calculated based upon the numbers of children and their characteristics in each school.
- informs ‘short term’ education policy monitoring and school accountability and intervention (for example, school GCSE results or Pupil Progress measures).
- supports ‘longer term’ research and monitoring of educational policy (for example how certain subject choices go on to affect education or earnings beyond school)
All data is transferred securely and held by the Welsh Government under a combination of software and hardware controls.

Requesting access to your personal data
Under data protection legislation, parents and pupils have the right to request access to information about them that we hold. To make a request for your personal information, or be given access to your child’s educational record, please contact the school office directly.
In certain circumstances you also have the right to:
- object to processing of personal data that is likely to cause, or is causing, damage or distress
- prevent processing for the purpose of direct marketing
- object to decisions being taken by automated means
- in certain circumstances, have inaccurate personal data rectified, blocked, erased or destroyed; and
- a right to seek redress, either through the ICO, or through the courts
